Based on observations made and discussions held during the inspection, the following action items from the inspection require your attention: Philbrook Dams: 1.Sloughing was observed on the steep side slopes along the upstream approach channel of the radial gate spillway. Our site observation noted that they could become potentially unstable from a heavy precipitation event. Evaluate the conditions and determine if repairs or mitigation measures are warranted. 2.Significant vegetation was observed on the upstream face and crest of both the Main and Saddle dams, as well as the downstream face of the Main dam. In some places the height of the vegetation exceeded more than 4 feet. Remove the vegetation and enhance the vegetation maintenance program to maintain the removal of all woody vegetation and trimming all other vegetation to aid in visual observations. Note that the removal of vegetation that would result in ground disturbing activities will need to be submitted to this office for review and authorization prior to performing the work. 3.In the downstream channel of the spillway with flashboards,about 100 ft away from the lip, there is a void approximately 2 feet wide by 2 feet deep due to potential displacement of a boulder. Evaluate the conditions and determine if repairs are warranted. Document Accession #: 20260720-3015 Filed Date: 07/20/2026 2 Round Valley Dam: 4.Concrete spalling was observed on the crest of the spillway. Additionally, a triangular-shaped void, approximately 6 inches wide and 6 inches deep, was observed near approximately 1/5 of the spillway crest length away from the right wall. Evaluate the conditions and determine if repairs are warranted. 5.Drain 4 pipe, located next to a large rock near the downstream toe, was deformed. Our visual observation noted that the crown of the drain pipe is flattened,and the side walls are missing near the exit.Evaluate the conditions and determine if repairs are warranted. 6.Sloughing was observed on the side slopes of the downstream channel of the spillway.Our observation noted that they could become potentially unstable during a heavy precipitation event. Evaluate the conditions and determine if repairs or mitigation measures are warranted.
